Unique in the nation, Benedictine’s ER Senior Care Coordinator assists senior ER patients and their family or significant other with the coordination of home care services, residential care or alternative care solutions and financing options.
The ER Senior Care Coordinator works with the care coordinators in the hospital. When a patient is admitted, vital information on the patient is given to the care coordinator in order to make the discharge process easier. After discharge, the coordinator then follows up with calls to the patient’s home to address any needs, which may have been overlooked.
